The service is open for those aged 3 to 18 years. All young people will have an assessment to create their care plan. We match the staff to the young person, taking into account the young person's needs, interests and their parent/carers requirements. Parents/carers, social workers and family workers can refer young people to the service. This service is open to all young people who have the funding, budget, direct payment or package to purchase the service. Parents/carers decide what times and days they need support and then book these with the home service lead.
